This is a Signup/Authorization form that you can display at your front desk for field trips and activities. The report displays in only one format, however, it effectively can be used for two different purposes depending on when it is printed.
If you assign the students to the activity after printing the report it will basically be a standard "blank" signup and Authorization form for your parents. The report will contain information about the trip in the header area of the report and then have blank lines below so Guardians can signup their children and apply their signatures.
If you assign the students to the activity before printing the report it will contain a page before the "blank" signup section that contains a list of all these pre-enrolled students. This is useful if you have a predefined number of students targeted for the trip (e.g. only the clubhouse, etc.). In this situation the parent would find their child's name on the report and apply their signature to authorize the trip.
