This log report is intended primarily for off-site field trips that involve transportation to and from the activity. This log can be printed out and given to the coordinator of the activity. It provides you with a pre-formatted "Checklist" for each student enrolled in the Activity with separate boxes for performing roll-call each time they get on and off the bus.
In addition, the report prints out vital statistics about the activity (address, telephone number, etc.) as well as the nearest Medical Facility. Lastly, it will display any comments or travel directions you have recorded in the Activity Folder of this activity so they can easily be read by the Bus driver and/or Activity Coordinator.
