The Required Forms Schedule allows you to define all the forms that are required to be on file at your school for each student. Once defined, SchoolLeader will track expiration dates of forms to ensure that all students are in compliance.
To access the Required Forms Schedule, click on Families, then Student Forms Compliance, then Edit Student Forms Schedule.
SchoolLeader provides a sample forms schedule, which must be updated to reflect your specific needs.
To add a form, type in the next available row designated with an '*'. You MUST complete one row before moving on to the next row.
To change a form description, type over the selected form description.
To delete a form, click in the 'gray area' of the row you want to delete. The row will become 'highlighted'. Now click on the DELETE key on your keyboard.
If you accidentally 'blank out' a form and you can't close this window, click on the ESC key on your keyboard.
Once you are done, you can click on the 'X' in the upper right hand corner of the Forms Schedule window to close this window. If any changes have been made to the Forms Schedule, you will be receive a SchoolLeader message stating that 'All Forms will now be refreshed'. Click on OK.
Type in the name of the form. There are no limits to the amount of forms that can be tracked.
Each form can be associated with three different student age groups. The three student age criteria and their definitions are as follows:
1) Infants - Students less than one year old
2) Pre-K - Students who are one year or older but are currently not attending any "Outside/Transporting Schools".
3) School Age - Students who are one year or older and are attending one or more "Outside/Transporting Schools".
SchoolLeader knows that a student is 'school age' because they are associated with an Outside/Transporting School. Refer to Defining Outside/Transporting Schools for more information.
The forms defined in this schedule will be automatically populated in each student folder, so you only need to enter this information once in this centralized location. Any changes made to the Forms Schedule will be reflected in every student folder.
For states or counties who need to track that a medical form is on file rather than tracking the individual vaccination shots, use this Required Forms Compliance Schedule to define your required medical form. If you need to track individual vaccinations, then refer to the Student Vaccine Schedule.
